INITIAL APPLICATION FOR ADMISSION

A completed application consists of:
*Completion of this form
*Proof of high school graduation or equivalency
*Provide proof of identity in the form of a state issued driver’s license or government issued ID
*$50 application fee (send fee via mail, pay through website via PayPal or pay with mastercard/visa over the phone at 319-462-3402)
Applicant is required to provide proof of high school graduation or equivalency.
